Portsmouth Ohio Restaurant Report Card

The information for the Restaurant Report Card comes from the Portsmouth Health Department. The results are in random order and not “ranked”. Not all restaurants are inspected every month. Health Code violations are separated by “Critical” and “Non-Critical”.

This report is not intended to besmirch any local business. Rather, we hope this is a learning tool for all food service workers and managers. Along with informing the public, please use this information to improve your knowledge and abilities in your careers. As my Grandpa said, many times, “Don’t let the same dog bite you twice”. New mistakes are expected. Don’t get caught up in a cycle of the same mistakes.

Taco Bell- Gallia Street

3717-1-04.1(Y) / Temperature measuring devices.
Observed thermometer not working properly and missing at walk in cooler.
All equipment should have a temperature measuring device to ensure correct holding temperatures. Correct by next inspection.
3717-1-04.2(I) / Sanitizing solutions – testing devices.
Observed expired sanitizer test kit.
Obtain in date sanitizer test kit to ensure proper sanitizer concentration. Correct by next inspection.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed build up of oil on hood system pipes and lights. Observed oil build up on fryer racks located next to hood and deep fryer.
All equipment should be kept in a good clean condition. Correct by next inspection.
3717-1-06.2(I)(1) / Lighting – intensity (10 FC)
Observed insufficient lighting in walk in cooler/freezer.
Fix lighting in the walk in cooler/freezer so that it provides 10 footcandles of light. Correct by next inspection.

Observed dust build up on dry storage in the back of the facility. Observed debris in the bottom of freezer in back of facility.
All equipment in the facility should be kept in good clean condition. Correct by next inspection.

Observed mops drying in mop bucket and in mop sink.
All mops should be allowed to properly air dry. Correct by next inspection.

Critical *Critical Violation* Observed water pooling on top of ice bin on drive-thru pop machine being able to drain back into ice bin once opened.
All food-contact surfaces should be kept clean to avoid possible contamination. Correct immediately.

Observed build up of debris on chemical storage rack. Observed debris in the bottom of reach-in freezer by the prep station by the office.
All nonfood contact surfaces should be kept in a good clean condition. Correct by next inspection.

Observed water pooling on the floor in the dry storage area from a condensation pipe under the hot water heater.
All liquid wastes should be properly disposed of. Correct by next inspection.
